APPLICATION FOR STUDENT PERMIT 

       For Filipinos

       Qualifications:

           1.      Must be at least sixteen (16) years old
           2.      Must be physically and mentally fit to operate a motor vehicle
           3.      Must be able to read and write the local dialect. Pilipino or English

            Requirements:

          1.      Original copy with photocopy of birth certificate. In the absence thereof,
                 any of the following:

                    a.      Joint affidavit of two (2) disinterested persons who know the
                          applicant and who can  attest to his/her age and identify
                          accompanied by non-availability of record;

                    b.      Any legal document to prove his age and identity such as SSS
                          ID, passport, or any government issued documents;

           2.   For applicants 18 years old and below, parent’s or guardian’s written
                 consent;

           3. One (1) ID picture;

           4. Duly accomplished Application for Driver’s License (ADL).

               Venue: Any District Office

           For Foreigners

         Qualifications:

           1.      Must know how to read or write in English or Pilipino
           2.      Must have been in the Philippines for at least one (1) month and can
                 show evidence that he will stay in the country for at least five (5)
                 months.

                  Requirements:

          1.      Original and photocopy of passport with visa duration of at least five
                 (5) months from  date of application and any of the following:

                       a.      Valid Alien Certificate of Registration (ACR);
                       b.      If born in the Philippines, original and photocopy of birth
                             certificate; and
                       c.      If student, original and photocopy of school ID.

          2.      Two (2) ID pictures;

          3.      Duly accomplished Application for Driver’s License (ADL)

                                 iii.      Pilot licensing Unit

                        For provincial Applicants
                                   i.      Regional Offices and/or

   APPLICATION FOR NON-PROFESSIONAL DRIVERS LICENSE
   
   New

   Qualifications:   

       1.      Must be at least seventeen (17) years old
       2.      Must be physically and mentally fit to operate a motor vehicle
       3.      Must be not a drug user or alcoholic
       4.      Must be able to read and write Pilipino local dialect or English
       5.      Must have a clean cut hair
       6.      Must be clean, neat and presentable (wearing Sando during
             photo-taking will not be allowed)

             Venue:
                  a.     For Metro Manila Applicants – Licensing Centers
                  b.     For Provincial Applicants – District offices

    Requirements:

1.      Valid Student Driver’s Permit at lest one (1) month old and with ID picture attached. For foreigners, photocopy of passport or ACR duly authenticated by the District officer/Licensing heads

2.      Medical Certificate duly signed by an accredited LTO physician or government physicians provided accomplished by official receipt (OR)

3.      Must have passed the written and practical examinations

4.      Duly accomplished Application for Driver’s License (ADL) 

      Change classification 

     Requirements: 

1.      Professional Driver’s License

2.      Current Drivers License Receipt (DLR)

3.      If dormant, require written and practical examinations

4.      Duly accomplished Application for Driver’s License (ADL)
