APPLICATION FOR PROFESSIONAL DRIVER’S LICENSE

    New    

    Qualifications:

        1. Must be at least eighteen (18) years old
        2. Must be physically and mentally fit to operate a motor vehicle
        3. Must not be a prohibited/dangerous drug user or alcoholic
        4. Must be able to read and write Pilipino, local dialect or English  
        5. Must have a clean cut-hair  
        6. Must be clean, neat and presentable. Wearing Sando and Slippers are
            not allowed

     Requirements 

1.     Valid Student Driver’s Permit at least five (5) months old with the Drivers License Receipt (DLR)

2.     Medical Certificate duly signed by an accredited LTO physician or by government physician provided accompanied by an official receipt (OR)

3.     Medical Certificate duly signed by a physician specializing on contagious diseases (Optional)

4.     Medical certificate duly signed by a physician on prohibited/dangerous drugs and alcohol (Optional)

5.     NBI or police or Fiscal's or Court clearance from place of residence

6.     Must have passed the written and practical examinations

7.     Duly accomplished Application for Drivers License (ADL)

Change classification

1.      Must be at least eighteen (18) years old

2.      Must be physically and mentally fit to drive

3.      Must not be affected with any contagious disease such as tuberculosis and sexually transmitted disease, epilepsy

4.      Must not be a prohibited/dangerous drug use or an alcoholic

5.      Must be able to read and write Pilipino, local dialect or English

  Requirements:

1.    Non-Professional Driver’s License at least four (4) months old

2.    NBI or police or fiscal or court clearance from place of  residence

3.     Must have passed the written and practical examinations

4.      Duly accomplished Application for Driver’s License (ADL)
a.      For Metro Manila Applicants – any licensing center
b.      For provincial Applications - any District Office   

    APPLICATION FOR CONDUCTOR’S LICENSE

    Qualifications:

      1.      Must be at least eighteen (18) years old

2.      Must be physically and mentally fit to work

3.      Must not be afflicted with any contagious disease such as tuberculosis or sexually transmitted diseases, epilepsy

4.      Must not be a prohibited/dangerous drug user or an alcoholic

5.   Must be able to read and write Pilipino local dialect or  English

6.      Must  have a clean cut hair. 

7.   Must be clean, neat and presentable. Wearing sandos and slippers are
not allowed

     Requirements: 

1.      Original Birth Certificate or any legal document to provide his identity and age

2.      Medical Certificate duly signed by an accredited LTO physician and /or by government physician provided accompanied by an official receipt  (OR)

3.      Medical Certificate duly signed by an physician specializing on contagious diseases

4.      Medical Certificate duly signed by a physician on prohibited/dangerous drugs and alcohol (Optional)

5.      NBI or police of Fiscal or Court clearance from place of residence

6.      Must have a passed the written examinations

7.      Duly accomplished Application for Driver’s License (ADL)

b.     For provincial Application – Any District Office

     Requirements:

        1. Current Conductor's license due to expire or expired but not more than
             but not more than two (2) years.
